The Mask Is Exhausting: The invisible cost of looking organized, capable, calm, and fine.

One of the reasons ADHD can be overlooked—particularly in women—is that what other people observe doesn’t necessarily capture the effort occurring underneath.

A person may arrive early, meet deadlines, maintain a career, use extensive organizational systems, or appear attentive while simultaneously putting significant effort into compensating for areas that are difficult.

Recent research continues to identify differences in ADHD presentation among females. A 2025 systematic review found that adult females with ADHD more frequently endorsed some experiences including distractibility, difficulty organizing tasks, mind wandering, and adverse impacts at home compared with adult males with ADHD.

What Does ADHD Masking Mean?

“Masking” is commonly used to describe attempts to hide, suppress, or compensate for ADHD-related difficulties.

Importantly, ADHD masking is still an evolving research area. Some researchers have cautioned against simply importing the established autism concept of camouflaging into ADHD without ADHD-specific definitions and validated measurement. 

At the same time, qualitative ADHD research has documented adults describing hiding symptoms and using substantial effort to appear more conventionally functional, while research with girls and young adults has identified strategies such as reminders, lists, arriving early, maintaining high work standards, subtle fidgeting, and suppressing visible behaviors. 

Visible Competence vs. Invisible Cost

A completed task tells us the task was completed.

It doesn’t tell us:

  • How long it took

  • How much anxiety preceded it

  • How many reminders were required

  • Whether sleep or meals were sacrificed

  • How much recovery was needed afterward

  • Whether the strategy is sustainable

That creates an important distinction:

Competence tells us what someone can produce. It doesn’t necessarily tell us what producing it costs them.

Compensation Isn’t Automatically Bad

Calendars, reminders, preparation, routines, environmental supports, written instructions, and organizational systems can be genuinely useful.

The goal isn’t to eliminate them.

Instead, consider asking:

Is this strategy supporting me—or am I using enormous amounts of energy to make sure nobody sees me struggle?

Support vs. Performance

Support: “Please send that to me in writing so I don’t lose it.”

Performance: Pretending you’ll remember because asking for help feels embarrassing.

Support: Using reminders because they reliably help.

Performance: Believing you shouldn’t need reminders and judging yourself every time you use one.

Understanding your brain doesn’t mean abandoning responsibility.

It can mean building responsibility around the way you actually function.

A Question to Try This Week

Choose something you’re known for doing well.

Then ask:

How am I accomplishing this?

What supports it?

What does maintaining it cost me?

You may discover that your strategy is working beautifully.

Or you may discover that appearing “fine” has required far more energy than anyone around you realizes.

Both pieces of information matter.
